PARENT/GUARDIAN CONSENT & PARTICIPATION AGREEMENT
 

Hueytown Gophers Youth Basketball
 

Welcome to the Hueytown Gophers Youth Basketball Program! We are committed to providing a safe, positive, and enjoyable basketball experience for young athletes while promoting teamwork, sportsmanship, discipline, and skill development.

 

1. Authorization to Participate

I certify that I am the parent or legal guardian of the child or children listed in this registration.

I voluntarily grant permission for my child or children to participate in Hueytown Gophers Youth Basketball activities, including practices, games, evaluations, and other official program events.
 

2. Acknowledgment of Risks

I understand that basketball is a physical activity that involves certain inherent risks, including falls, collisions, and injuries.

I acknowledge these risks and give permission for my child or children to participate in the program.
 

3. Medical and Emergency Authorization

I agree to provide accurate emergency contact information and notify program officials of any health concerns or participation restrictions that require reasonable accommodations.
 

In the event of an injury or medical emergency, I authorize program representatives to contact emergency medical services and seek appropriate assistance when I cannot be reached promptly.
 

4. Player and Parent Code of Conduct

I agree to encourage good sportsmanship and respectful behavior toward coaches, officials, teammates, opponents, and other families.

I understand that participants and spectators are expected to follow the program's established rules and conduct guidelines.
 

5. Registration and Payment Acknowledgment

I understand that the registration fee is $135 per child.

I confirm that the number of children included in my registration matches the number of registration fees being paid.

I understand that any refund or cancellation requests will be subject to the program's official refund policy, once provided and made available for review.
 

6. Confirmation of Information

I certify that all registration information submitted is accurate and complete to the best of my knowledge.

I understand that providing incorrect information, including a player's age or date of birth, may affect eligibility or team placement.
